This form is used to fill out an evaluation for a training.

ECE-TRIS Individual Training Form

If there are no ECE-TRIS sign-in sheets provided at an out of state conference OR you completed a non-face to face training, please use this form to submit to ECE-TRIS for entry for DCC approved trainings. Follow the instructions found on the form and submit to ECE-TRIS. DCC Training Approval Lists can be found by visiting the following link: http://chfs.ky.gov/dcbs/dcc/trng_app.htm
